Description

• Provides integrated support to patients, pharmacists, and various healthcare providers. • Works closely with patients and/or their families to obtain the best possible medication and allergy history. • Integrate and collaborate with the emergency department’s admitting clinical teams. • Operate electronic health records to obtain patient medication history information. • Document medication and allergy discrepancies in the patient medical chart. • Process orders using pharmacy computer system. • Assist in training PharmD students and pharmacy residents on the medication reconciliation process. • Maintains required documentation in accordance with regulatory and hospital requirements. • Generate necessary pharmacy reports. • Help develop new processes to ensure smooth operations and patient care.

Requirements

• High school diploma or equivalent. • Current license with the Massachusetts Board of Pharmacy. • Certified by one of the national governing bodies such as Pharmacy Technician Certification Board PTCB, or the National Healthcareer Association ExCPT. • Three years of previous pharmacy experience. • Proficiency in math with knowledge and ability to use a computer for data entry. • Must be able to follow oral and written instructions with accuracy, efficiency, and dependability. • Positive work ethic, motivated and professional demeanor to promote teamwork in the department. • Strong interpersonal communication skills.
