Description

• Lead a team to improve providers' performance under value-based contracts • Collaborate to manage patient populations and improve outcomes • Develop, implement, and optimize value-based care programs • Monitor and evaluate performance of value-based care initiatives • Build and maintain relationships with external partners • Drive market-level value-weighted membership growth • Partner with analytics to track performance of value-based care programs

Requirements

• Bachelor’s Degree in business, healthcare administration or a relevant field required • Master’s preferred • Minimum of 10 years of management experience in the healthcare industry • Proven track record of successfully implementing and managing Population Management and Value-Based Care programs • Strong leadership skills • Validated ability to implement data into practice • Prior experience handling a P&L or accountability for a large-scale project • Proficiency with Microsoft Excel and PowerPoint required • Experience with Electronic Medical Records (EMR) and work with healthcare data and reporting is strongly preferred • Must be willing and able to commute daily to practices throughout Arizona
