• At Diverge Health, we are a team of entrepreneurs passionate about improving health access and outcomes. • As the SVP of Population Health, you will lead the Population Health team in partnership with Diverge Health’s Chief Medical Officer. • You will play an influential role in shaping Diverge Health in its next phase and work cross-functionally to ensure we achieve our goals.

Requirements

• 15+ years of experience leading and growing teams in fast-paced healthcare companies. • Significant experience working closely with care teams serving patients on Medicaid and Medicare. • Demonstrates a strong understanding of value-based care principles, including the impact on patient outcomes, quality, and cost. • Extensive experience leading the development and implementation of innovative patient care programs. • Track record of synthesizing vast amounts of information, prioritizing work accordingly with an eye to near-term feasibility, and collaboratively partnering with operations, technology, people, and finance teams. • Strong attention to detail, analytical, effective communication skills, and a collaborative mindset. • An ability to travel to our markets on an as-needed basis. • 5+ years in management consulting and/or corporate strategy roles, demonstrating a proven ability to produce high-quality deliverables, communicate effectively with executive leadership, and exhibit strong executive presence in high-stakes environments.
